Are you an existing user who is trying to add additional investigators or study staff to a protocol? a) First fill out the form below. b) Then use IRBWise to submit an amendment adding the additional investigator(s) or study staff member(s).
1) Send an electronic copy of your curriculum vitae (in pdf, rtf, html, or Word format)
2) Send a copy of your CITI training certification (in pdf, rtf, html or Word format). To complete the CITI course, please see "Education and Training" (See DOH IRB Policy "Investigator and Key Study Personnel Training)
3) Complete the following information (* indicates required field) and click "send information" below. Note: you should receive your username and password within 2 business days via email. Please contact the office to confirm your email if you do not see a response
